As a patient, you have the right: |
- To considerate and respectful care.
- To receive, upon request, the name of the physician responsible for your care and to know the identities of others involved in providing your care.
- To obtain from the physician complete and current information concerning your medical diagnosis, its likely course, contemplated treatment and outcomes in terms and language that you or a designated person can be expected to understand.
- To designate a representative to participate in health care decisions with you or on your behalf.
- To receive information necessary to give informed consent prior to the start of your procedure. This information shall include, at the minimum, an explanation of a specific procedure or treatment itself, its value and significant risks, and an explanation of other possible treatments, if any.
- To know of any experimental research or educational activities involved in your treatment and the right to refuse to participate in any such activity.
- To privacy and confidentiality of all records pertaining to your treatment, except as otherwise provided by law or third-party payment contract.
- To refuse treatment to the extent permitted by law and to be informed of the medical consequences of such an action.
- Upon request, examine and receive an explanation of your bill, regardless of the source of payment.
- Treatment without discrimination as to race, creed, color, religion, sex, national origin, or handicap.

- To have your pain issues addressed pre and post-procedure.

Provision of Information: To provide, to the best of your knowledge, accurate and complete information about your present complaints, past illnesses, hospitalizations, medications, and other matters relating to your health. You are responsible for reporting whether you clearly comprehend a contemplated course of action and what is expected of you.
Compliance Instructions: To follow the treatment plan recommended by your health care provider. You are responsible for keeping appointments and notifying your physician when you are not able to keep the appointment.
Refusal of Treatment: You are responsible for your actions if you refuse treatment or do not follow the practitioner’s instructions.
Center Charter: To assure that the financial obligations for your health care are fulfilled as promptly as possible.
Center Rules and Regulations: To follow Center rules and regulations affecting patient care and conduct.
Respect and Consideration: To be considerate of the rights of other patients and Center personnel and for assisting in the control of noise, smoking, and the number of visitors. You are responsible for being respectful of the property of other persons and the Center. |
