SERVICES AT THE CENTER:

Nondiscrimination
Our Center provides treatment to all patients without regard to race, color, creed, religion, or national origin. We are also an equal opportunity health care provider and employer, regardless of handicap, in accordance with the Rehabilitation Act of 1973, Section 504, and the Americans with Disabilities Act of 1990.

Safety
During your stay at our Center, your safety is of great concern to us. To prevent accidents, please call a nurse for assistance. Use the utmost care when getting on and off stretchers, chairs, or wheelchairs. Our safety program requires that we hold regular fire, disaster, and safety drills. If this occurs during your stay with us, you will be informed.

Smoking Policy
In accordance with the Florida Clean Indoor Air Act, our center is designated a non-smoking facility. There are designated smoking areas outside the building.

Consent for Treatment
A consent form for urological and anesthesia treatments must be signed by each patient or their next of kin. Permits for minor children must be signed by a parent or legal guardian.

Living Will/Advance Directives
Since our Center is an out-patient facility for the treatment of urologic stone disease and BPH, it is the policy of our Center that, in this "ambulatory" setting, any advance directive be presented by the patient at the time of admission. Health Care Surrogate and Legal Power of Attorney will be honored at the Center and will become part of the medical record and reviewed with the patient or his/her surrogate decision-maker, as appropriate in an ambulatory care setting.  We do not honor “do not resuscitate” requests.

Financial Arrangements
A representative from our business office will be happy to answer any questions you may have about your bill. They will also help file a claim to your insurance company. You will be advised of your insurance benefits. Remember that your insurance company is obligated only to you, the policyholder, and you are ultimately responsible for your bill.

You should bring your insurance card and drivers license with you at the time of treatment.

Co-payments are due prior to treatment. If you require the services of a urologist, radiologist, anesthesiologist, pathologist, cardiologist, or any other specialist for the interpretation of tests, their bills will be submitted separately, and payment should be made to them directly.

Your account may be settled by cash, check, Visa, or MasterCard. Prompt payment helps control the cost of medical care. Please contact our Billing Office at 727-521-3929 ext 235 or email pchornoby@barsc.com if you need to make special financial arrangements.

Medicare patients will be registered through St. Anthony’s Hospital.  Although the billing will be done by the Hospital, all admission forms will be signed at Center.  There is no need to visit the Hospital prior to your treatment.  Any co-insurance due will be paid directly to St. Anthony’s Hospital.
